JFSA Community Care at Home, Skilled Home Health Agency

Community Care at Home is seeking a full-time RN Manager of Clinical Services to establish and maintain direction, operation practices, procedures, and standards for Skilled Nursing Services provided by CCAH.

The RN Clinical Manager manages the clinical aspects of Skilled nursing/Medicaid/Medicare services provided by field and contracted staff; partly manages this program’s operational budget and reviews electronic documentation (OASIS and 485’s) to ensure accuracy, quality and cost effective delivery of services.


Responsible for addressing client care concerns and evaluate the effectiveness of clinical services - providing training and in-service opportunities to ensure continuous quality improvement.
Manages intake for skilled nursing referrals by working collaboratively with CCAH managers/staff to initiate unskilled Medicaid services, to ensure the timely scheduling of the start or recertification of skilled nursing services, and to ensure appropriate follow up with individual clients and referral sources.


Coordinates documentation and reporting of complaints and unusual incidents occurring with skilled nursing patients and/or employees working collaboratively with CCAH managers/staff to produce subsequent plans of correction.


Participates in the Administrator on-call schedule to provide administrative after hours support.

The right candidate for the job must be a graduate from an approved school of professional nursing (BSN preferred) and have a valid RN license in the State of Ohio. Must have a minimum of 3 years of nursing experience in the health care industry and 1 year of experience in Home Care. Must possess at least 2 years of management and supervisory experience within a home health or social service agency. Must have a valid driver license, insurance and transportation as well as a clean driving record. All applicants must understand the issues related to the delivery of home health services and be able to problem solve effectively, possess knowledge of the Medicare guidelines governing home health agencies and possess excellent communication, management and organization skills, be detail oriented and able to work with a diversity of individuals.
